How to Refer a Patient
01. Submit Referral
Send patient information via Fax: 410-841-6030 or use our Secure Email portal. Our intake team is available for immediate consultations at 410-803-2600.
02. Required Documentation
Please include the most recent History & Physical (H&P), current medication list, insurance details, and specific physician orders for skilled services.
03. Clinical Review
Once documentation is received, a Registered Nurse will conduct a clinical assessment within 24 hours. We coordinate directly with the family to initiate care.
